You should return to Cottonsoft: -
<p>
1. The completed Statutory Declaration form - on this form you have to circle any of the statements that apply to you. This form must be witnessed by a person from the list shown on the back of the form. Read carefully the notes on the second page of this form regarding the points you should circle.
<p>

2. The enrolment form accompanied by proof of your identity, citizenship/ residency and age (Australian birth certificate, birth certificate extract, Australian passport, naturalisation certificate, green Medicare card or appropriate visa {this may include a Visa Evidencing Card that demonstrates a protection visa} that has been witnessed and signed by a Justice of the Peace). By completing this enrolment form, you are authorising Cottonsoft to apply for funding on your behalf. You are also agreeing to post the Training Voucher to Cottonsoft immediately you receive it. This must be received by Cottonsoft within 14 days of you receiving it or you may not be able to start the course.
<p>
3. Either:-<br />
If you are currently employed, you need to provide a copy of a recent payslip.<br />
If you are an income support recipient receiving Personal Support, Disability Support, Vocational Rehabilitation Support, Parents or Carers payments you will need to supply a copy of an Income Statement and current Activity Agreement from Centrelink. If you are in receipt of a parenting allowance as your income you will need to send a copy of the birth certificate of your youngest child. <br />

If you are an unemployed job seeker, you will need to supply an Income Statement and current Activity Statement from Centrelink.

4. The completed Literacy and Numeracy tests. (If you feel you have limited skills in reading that could affect doing the course please let us know and we will assist you before you start the course itself).

Please Note: You are not required to travel to attend classes. We provide 7 day a week phone and email support while you are completing your distance education coursework. Note that if you already have some of these skills, we can give you credit for them.

The choice of when to study and how hard you work is up to you. The distance education (correspondence) format allows you to work at your own pace. You do need a business you can use as a case study, someone you can ask questions about their business that will assist and enable you to successfully complete activities in each unit. If you undertake computing skills units ready access to computer is also essential.
